Repeat Violation. 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Raw chicken & raw fish stored over cooked ready to eat foods in the 1-door reach-in cooler. Store foods according to their cook temperatures and/or bacteria load. PIC was advised on the proper storage of foods and foods were re-arranged. Corrected On-Site. New Violation. 4-2B, Violation of Code: [.05(6)(o) ] Employee actively washing equipment/utensils without sanitizing equipment (sink was not set up for proper wash, rinse & sanitize). Equipment should be washed, rinsed & sanitized and be clean to sight/touch to prevent bacteria growth. Three compartment sink was properly set up and equipment/utensils were re-washed, rinsed and sanitized before being placed to air dry. Corrected On-Site. New Violation. 6-2, Violation of Code: [.04(6)(g) ] Three container of cooked chicken tamales cooked on 02/12 not date marked with the date of preparation and/or date product must be discarded. Cooked potentially hazardous foods should be marked with the date of preparation and/or date product must be discarded. Day one is the date of preparation, plus six days for a total of seven days before the product must be sold, consumed and/or discarded. Containers were date marked. Corrected On-Site. Repeat Violation.

Violations: 8-2B, Violation of Code: [.07(6)(b) ] Several spray bottles of cleaners and chemicals were found with no common name labeled on the containers. The person in charge placed labels on these containers. All chemicals removed from original packaging and placed into working containers should be labeled with the common name of the chemical. Corrected On-Site. New Violation. 12A, Violation of Code: [.04(4)(u) ] Sliced lemons for self service near the drink station were found in an unprotected container. The lemons were removed. All items on display such as lemons were must be properly protected from contamination via sneeze gaurds, individual containers etc. Corrected On-Site. New Violation. 15B, Violation of Code: [.05(3)(h),(i) ] Quat sanitizer used in the facility and no test strips available at the time of inspection. Test strips should be available for all sanitizers used in the facility. New Violation. 16A, Violation of Code: [.06(1)(j) ] Hot water not available at the men’s and women’s restrooms at the time of inspection. The person in charge was advised to have the hot water system repaired and ensure that water of at least 100 degrees is available at the hand washing sinks. Repeat Violation.

Facility replaced sanitizer solutions - one bucket for QUAT and the other with chlorine. Facility uses both for different tasks. Must maintain adequate level of sanitizer for effectiveness.
