Violations: 4-2B, Violation of Code: [.05(7)(a)1 ] Waffle iron, trays for cooling, tomato dicer and potato spiraler observed stored with food debris on them. All food contact surfaces must be stored clean to sight and touch. All unclean items were taken to dish machine area to be washed. **Please note this is a repeat violation and if it is marked as a violation on the next routine inspection the food service permit may be suspended.**  Corrected On-Site.  Repeat Violation. 5-2, Violation of Code: [.04(7)(e) ] Disclosure that eggs, salmon, and hamburgers are cooked to order not listed on menu. This was marked as a note on the last 2 routine inspections. Guide on how to properly mark menu was given at last routine inspection. All items that are served raw or undercooked must include a disclosure on menu that those items may be served to the customers preference or cooked to order. **An informal inspection will be done WITHIN 10 days to ensure that menu has been corrected.**  New Violation. 14D, Violation of Code: [.04(4)(n) ] Observed employee use gloved hand to season steak to put on grill then proceed to touch coolers and bagged sauces. Gloves must be changed when going from task to task, any other time when they are contaminated, or when they are in disrepair. Employee was advised and gloves were changed.  Corrected On-Site.  New Violation. 15A, Violation of Code: [.05(6)(b) ] Cutting boards on cook line observed with heavy cuts and discoloration. All cutting surfaces must be maintained in good repair by either being replaced or resurfaced. **Please note this is a repeat violation**  Repeat Violation. 15A, Violation of Code: [.05(6)(a) ] Rusted shelving observed in walk in cooler, dry storage area and in prep coolers and freezers. All equipment must be maintained in good repair. Please replace or paint with a high gloss/epoxy paint. **Please note this is a repeat violation**  Repeat Violation. 15C, Violation of Code: [.05(7)(a)2,3 ] Tables under grill and sides of fryers observed with food and grease buildup. All equipmemt must be cleaned at a frequency to prevent accumulations.  New Violation. 17C, Violation of Code: [.07(5)(a),(b) ] Floors and walls under and behind equipment observed with grease and food debris buildup. All physical facilities must be cleaned at a frequency to prevent accumulations which may attract pests.  New Violation.

Violations: 3-1C, Violation of Code: [.04(1) ] Observed small live bugs in rice container located by hand washing sink. Rice was discarded. Foods should be safe, unadulterated, and honestly presented  Corrected On-Site.  New Violation. 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Observed raw eggs stored over carrots and cauliflower in walk-in cooler. Observed raw shrimp stored behind green pepper in the top of prep cooler. Observed pooled eggs stored over sauces in bottom of 2-door prep cooler. All items rearranged accordingly. Food shall be protected from cross contamination by: 1) storing raw animal foods below/ in front of /away from produce/ cooked/ ready-to-eat foods; 2) storing/ separating raw animal foods by the minimum cooking temperature; 3) storing unwashed produce below/away from washed produce, ready-to-eat/ cooked foods; 4) only doing 1 task at a time in the dish sink example) a) thawing food or b) washing/ rinsing/sanitizing dishes; 5) preparing raw animal foods away from ready-to-eat foods, utensils, equipment, linens; 6) 4) using separate utensils (example: 1 for ready-to-eat foods, 1 for chicken, 1 for seafood, 1 for beef, etc).  Corrected On-Site.  New Violation. 4-2B, Violation of Code: [.05(6)(n) ] 1.)Dish machine not dispensing adequate concentrations of sanitizer, dishes being washed at time of inspection. 2.)Observed employee rinse knives off with water and then place knives with clean knives, without using proper washing procedures.
Knives were sanitized at time of inspection. Dish machine sanitizer needs to be repaired. Until dish machine is producing sanitizer facility will manually sanitize dishes. Food contact surfaces should be cleaned and sanitized with adequate levels of sanitizer.  Corrected On-Site.  New Violation. 14B, Violation of Code: [.05(10)(a) ] Several containers "wet-nested". After washing and sanitizing dishes should be air-dried before stacking.  New Violation. 14D, Violation of Code: [.04(4)(n) ] Single-use gloves not discarded when damaged or soiled, or when interruptions occurred in the operation. Observed employees using the same gloves for multiple task. (handling food, handling dirty then clean dishes, handling equipment) Had employees change gloves. Single-use gloves should be discarded when damaged or soiled, or when interruptions occurred in the operation  Corrected On-Site.  New Violation. 15A, Violation of Code: [.05(6)(a) ] Several fryer baskets in disrepair. Repair baskets to prevent physical hazards.  Corrected On-Site.  New Violation.
