Food contact surfaces need to be clean to sight and touch, please clean thoroughly and more frequently to prevent this accumulation. Nozzles were cleaned during the inspection. Corrected On-Site. New Violation. 6-1A, Violation of Code: [.04(6)(f) ] In the prep cooler several food potentially hazardous food(PHF) items werre above 41oF. PHF must be maintained at 41oF or below at all times. Food items were discarded. Corrected On-Site. Repeat Violation. 12B, Violation of Code: [.03(5)(i) ] Two employee working with food had long beards and did not have on proper hair restraints. Employee with hair or beards longer than 1/2 inch must wear beard guards. Person in charge retrieved beard guards. Corrected On-Site. Violations: 2-1B, Violation of Code: [.03(5)(c) ] 1. Food employees not cleaning hands immediately before engaging in food prep. Food prep employees not washing hands immediately before putting on new pair of gloves to begin food prep. 2. Employees not washing hands after handling dishes or working in dishwash area before beginning food prep. 3. Employee not washing hands upon returning to the kitchen after leaving the kitchen area. Employees were advised of appropriate times to wash hands and proper handwash procedure. Hands must always be washed before beginning food prep, before donning a new pair of gloves, anytime hands have become soiled by dishwash or other activites, upon returning to the kitchen area. Could not answer all questions pertaining to the employee health policy. Explained policy to CFSM and gave him a copy of the employee health policy in a more "easy to read" version. All potentially hazardous foods (PHF's) in top of prep cooler #1 were above 41 degrees. Temperatures documented in temperature observation chart on report. All foods that were out of temperature were discarded by CFSM. Air temp of cooler was checked and it was 41 degrees. Advised CFSM to turn down temperature of cooler. Temp was turned down during inspection and reached 38 degrees by end of inspection. Foods were also being stacked too high in cooler and top lid was being left open. Explained to CFSM that these things also contribute to the temperatures being out and to make appropriate corrections to procedures. Corrected On-Site. Whole pizzas on racks for pizza by the slice sales were not written on time control records chart. Pizza have not been written down for the past 2 days. Times are also not being recorded correctly. time is written down from 11 - 3 everyday instead of writing down the actual time the pizza comes out of the oven and the actual time the pizza is sold or discarded. Time control procedures not written in advance and kept on site. Gave time control procedures to CFSM and explained how to fill out. Corrected On-Site. Cooked potatoes being cooled in beer cooler in a deep pan with plastic wrap tightly sealed on top. Foods needs to be cooled in shallow pans, with ice baths and/or with plastic wrap lightly covering food so the food can vent and cool properly. Plastic wrap was loosened on the potatoes so they can cool more quickly. Corrected On-Site. New Violation. 15A, Violation of Code: [.05(1)(a) ] Cardboard is being used to line shelf in dishwash area; cardboard is not a washable approved material; please remove cardboard. Liner must be smooth, non-absorbant, washable, easily cleanable. Clean the inside lide of prep cooler #1; lid has old food debris on it. 2. Clean inside of utensil container by hot hold units. 3. Clean inside of chopper parts container. 4. Clean the area around the handwash sink faucets. Keep all equipment food and nonfood contact surfaces clean. 5. Tighten and clean the handle of the beer cooler; old food debris on beer cooler handle. The floor is made out of wood and is in disrepair. The walls are made out of drywall and cement blocks. Neither are washable. The ceiling has water pipes running above the single-service items (paper cups, plates, to-go boxes, napkins, etc.) The floor must be replaced so that it is smooth, washable, non-absorbant and easily cleanable. You may install ceramic tile, quarry tile, VCT tile, commerical vinyl flooring. The walls must be painted with epoxy paint or high gloss paint. Extended time will be granted to complete this project. The facility has until 12/3/12 to complete. If violation is not correct by this date, a citation will be issued to the facility.
