Violations: 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] 1.Raw shrimp and salmon stored over onions and carrots and raw pooled eggs streod over carrots in prep cooler. Raw shrimp over milk, tomatoes and sauce and raw beef stored over cooked crawfish in 3 dr cooler. Raw chicken stored over raw pollock, raw snapper stored over tortillas and raw squid stored over cooked crawfish in walk in freezer.  Advised PIC that all raw animal products must be stored according to minimum cook temperature and must be stored below ready to eat foods. All coolers and freezers were rearranged during inspection. 2. Observed employee handle raw catfish for fryer then handle potatoes without changing gloves. Gloves must be changed when going from raw to ready to eat to avoid cross contamination. Potatoes were fried to 189F to kill possible pathogens. Corrected On-Site. New Violation. 5-2, Violation of Code: [.04(7)(e) ] Menu has been updated since opening inspection. NY Steak and Ribeye now offered undercooked to customers on menu. No consumer advisory, disclaimer or reminder. Provided documentation on how to properly label menu for foods that may be served raw or undercooked. New Violation. 10D, Violation of Code: [.04(4)(d) ] Spices and oils taken from their original container not labeled with common name. All foods taken from their origina container must be marked with the common name. Please place labels on all container including squeeze bottles. New Violation. 10D, Violation of Code: [.04(3)(k) ] Clams taken from their original container and stored in metal pan. Clams are purchased to local market in the wholesale section. They are packaged in a net bag with tags attached. Advised PIC that all shellfish must remain in their original container until finished. New Violation. 10D, Violation of Code: [.04(3)(l) ] No shellfish tags provided for clams. Shellfish tags must be held in facility for 90 days in chronological order. New Violation. 10D, Violation of Code: [.04(7)(b) ] Facility using both Swai and Pollock for Whitefish on the menu. All foods must be honestly presented. Advised to put the name of fish next to Whitefish on menu. Example: Whitefish (Swai or pollock).
