Violations: 6-1C, Violation of Code: [.04(6)(d) ] Two pans of re-fried beans cooling since 06/06 at 49 F in the walk-in cooler. PHF's must be cooled from 135 F to 70 F within two hours and from 70 F to 41 F in an additional four hours to prevent bacteria growth. Re-fried beans were discarded during inspection. Corrected On-Site. New Violation. 11A, Violation of Code: [.04(6)(e) ] Re-fried beans cooling in deep pans with tight fitting lids. PHF's should be cooled via shallow pans with loose coverings, ice baths, ice paddles or with ice as an ingredient. PIC was advised of proper cooling methods. Corrected On-Site. New Violation. 11C, Violation of Code: [.04(6)(c)] Raw beef thawing in bucket of water in the meat sink. Foods should be thawed under running water, in the refrigerator/microwave or by the cooking process. The raw beef continued thawing under running water. Corrected On-Site. New Violation.

Violations: 4-2B, Violation of Code: [.05(7)(b) ] Observed lemon slicer and the dicer put away clean with food debris on them. Advised PIC that both pieces of equipment must be broken down and clean more often. PIC broken down and put in the warewashing machine. Corrected On-Site. New Violation. 12C, Violation of Code: [.05(10)(b) ] Observed 3 sanitizer buckets without any sanitizer. Advised PIC must check buckets after they are filled to make sure the dispencer is dispencing. PIC replaced and filled more buckets. Corrected On-Site. Repeat Violation. 15B, Violation of Code: [.05(6)(p) ] At time of inspection PIC did not have the proper test strips to test the chemical sanitizer. Advised to get the proper test strips. New Violation. 4-2B, Violation of Code: [.05(7)(a)1 ] Ice machine has black mold-like substance present and slicer and knives stored in knife holder have dried food debris present. New Violation. 12D, Violation of Code: [.04(4)(g) ] Observed raw fish being thawed in the vegetable sink. Advised PIC that vegetable sink shall be used for prepping vegetables and fruits only. New Violation. 15A, Violation of Code: [.05(1)(a) ] Door of walk-in cooler not closing all the way due to ice formation around it and shelving in warewash area is rusted. Advised PIC to repair freezer door making it close all the way and to paint or replace shelving in warewash area. New Violation. 17C, Violation of Code: [.07(5)(a),(b) ] Floors especially around fyers, in the liquor room and under dishwasher)and walls (especially next to drink syrups)have a buildup of soil, grease and/or syrup present. Advised PIC to clean walls and floor to prevent attraction of insects. Repeat Violation. 17D, Violation of Code: [.07(2)(i) ] Light in warewash area is missing shielding. Advised PIC to replace shielding. New Violation. 18, Violation of Code: [.07(5)(k) ] Observed several flies in the kitchen area. Advised PIC to control flies by using an exterminator or other measures. New Violation.
