Violations: 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Employee handled a fryer basket handle (that had previously been used by another employee handling raw chicken) and then handled hamburger buns. Had employee discard buns, wash hands, and put on new clean gloves. Must keep food separated and protected during storage and preparation of food. Corrected On-Site. New Violation. 5-1B, Violation of Code: [.04(5)(f) ] Reheating chili on a hot plate was at 121F. The time the chili was placed on the plate exceeded 2hrs. The chili was discarded. Time for reheating for hot-holding should not exceed 2 hours. Corrected On-Site. New Violation. 6-1A, Violation of Code: [.04(6)(f) ] Shelled eggs, and sliced cheese being held in the reach-in cooler had a temperature over 41F. Potentially hazardous foods shall be maintained at 41F or below. New Violation. 14D, Violation of Code: [.04(4)(n) ] Employee squeezed a sanitizer towel and immediately picked up some ready to eat buns. Buns were discarded. Employee washed hands and put on new gloves. If used, single-use gloves shall be used for only one task as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation. Violations: 2-1B, Violation of Code: [.03(5)(c) ] Employee on cell phone while preparing chicken and did not wash hands prior to continuing preparation. Had employee wash hands and put on new gloves. Employee must wash hands when handling cell phone and going directly to cooking. Corrected On-Site. New Violation. 6-1C, Violation of Code: [.04(6)(d) ] Container of rice in the walk-in cooler was cooling over 2 hours, temp was at 77F. Item discarded. Cooked potentially hazardous foods should be cooled from 135oF to 70oF within 2 hours. Corrected On-Site. New Violation. 11A, Violation of Code: [.04(6)(e) ] Rice being cooled in large, tightly covered containers. 
Potentially hazardous foods should be properly cooled by effective methods (shallow pans, ice water bath, stiring the food in a container placed in an ice water bath, etc.)  New Violation. 12B, Violation of Code: [.03(5)(g) ] Employees preparing food with more than one plain band. Rings removed. Employees should only wear a plain ring such as a wedding band while preparing food. Corrected On-Site. New Violation. 14B, Violation of Code: [.05(10)(e) ] Utensils stacked wet after washing and not allowed to air dry. Items were placed at the 3 compartment to be re-washed. Clean equipment and utensils shall be stored in a self-draining position that allows air drying. Corrected On-Site. New Violation. 15C, Violation of Code: [.05(7)(d) ] Metal pans located with clean dishes had food splatter on the outside of and a metal container which holds spatulas, dipping spoons, and etc., had residue in the bottom. Items were placed by the 3 compartment sink for re-washing. Nonfood-contact surfaces of equipment should be kept free of an accumulation of dust, dirt, food residue, and other debris.
