Violations: 1-2, Violation of Code: [.03(2)(a)-(n) ] Person in charge failed to enforce managerial task such as: utilizing proper cooling techniques, proper cold holding for safety, proper sanitizing, prevention of contamination, ensuring food is unadulterated, and package integrity is in good condition. The person in charge must review CFSM course material or designate another employee to do these tasks. The person in charge must perform these tasks in order to gain active managerial control to reduce risk of foodborne illness.  New Violation. 3-1C, Violation of Code: [.04(1) ] Produce in walk in cooler starting to mold. The food items were discarded. Foods must be in good condition, safe, and unadulterated.  Corrected On-Site.  New Violation. 3-1C, Violation of Code: [.04(3)(e) ] Three canned goods were found with dents in the top or bottom seam mixed in with other can goods. The food cans were isolated from other dry goods. Food packages must be in good condition to protect the integrity of the contents inside from contamination. 2ND REPEAT VIOLATION OF CODE PROVISION.  Corrected On-Site.  Repeat Violation. 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] 1. Unwashed mint, green and red bell peppers over chimichurri sauce in walk in cooler. 2. Unwashed tomatoes over washed and peeled onions in walk in cooler. The foods were reorganized. Foods must be protected from cross-contamination during storage and preparation by storing unwashed produce under ready to eat foods.  Corrected On-Site.  New Violation. 4-2B, Violation of Code: [.05(6)(n) ] Dish machine observed cleaning dishes without dispensing any sanitizer. The machine was primed with available sanitizer; the dishes were rewashed. Utensils and food-contact surfaces must sanitized via mechanical operations that that expose the surfaces to the sanitizer for the appropriate amount of time.  Corrected On-Site.  New Violation. 4-2B, Violation of Code: [.05(7)(b) ] Blue Moon Beer tap and drink dispenser at bar observed with mold forming. The equipment were cleaned. Food contact surfaces must be cleaned with a frequency to prevent accumulation of food debris or mold.  Corrected On-Site.  New Violation. 6-1C, Violation of Code: [.04(6)(d) ] Several foods observed in the walk in cooler and on rack in prep area that had been cooling for 2+ hrs that had not reached 70°F. The goods were discarded. Cooked potentially hazardous foods must cool from 135°F to 70°F in 2hrs; from 70°F to 41°F or below in 4hrs.  Corrected On-Site.  New Violation. 6-2, Violation of Code: [.04(6)(g) ] Several potentially hazardous foods made onsite were observed without labels. The foods were labeled. Potentially hazardous foods being held in the facility for more than 24hrs must be date labeled with item name and dates to not exceed 7 days.  Corrected On-Site.  New Violation. 11A, Violation of Code: [.04(6)(e) ] 1. Container of garlic and oil mixture observed nesting inside another container in prep cooler. 2. Shredded cheese nested inside sliced cheese in prep cooler. 3. Three containers of meat foods and one container of rice observed in large bins in the walk in cooler. 4. One container of rice cooling at room temperature in large bin. The food items were discarded. Cooling must be done using methods approved by the health authority (i.e., ice baths, smaller portions, flat layers on thin sheet pans, ice wands, using ice as an ingredient, etc).  Corrected On-Site.  New Violation. 15A, Violation of Code: [.05(6)(b) ] Several cutting boards observed with deep gouges observed in them. The cutting boards were discarded. Cutting surfaces that are subject to scratching and scoring must be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.  Corrected On-Site.  New Violation. 15C, Violation of Code: [.05(7)(a)2,3 ] 1. Several metal pans observed with foods debris/residue on the underside of container rim. 2. Reach in cooler had food debris on the right bottom side of it. These surfaces were cleaned. Violations: 2-1B, Violation of Code: [.03(5)(c) ] Employee observed discarding trash outside and then reentered the facility and started working with cookie dough without first washing hands. The employee discarded dough and washed hands before continuing to work with food. Hands must be properly washed before working with food when they become contaminated such as taking out trash, leaving facility etc.  Corrected On-Site.  New Violation. 2-2D, Violation of Code: [.06(2)(o) ] The back hand washing sink was blocked at the time of inspection. The person in charge moved the equipment that was blocking the sink at the time of inspection. All hand washing sinks must be kept accessible for use at all times so hands can be properly washed.  Corrected On-Site.  New Violation. 10D, Violation of Code: [.04(4)(d) ] Large containers of sugar and flour not indentified with common name of food item at the time of inspection. The person in charge properly labeled these items at the time of inspection. All food storage containers must be labeled with common name of food if item is not easily identifiable.  Corrected On-Site.  New Violation. 10D, Violation of Code: [.04(3)(l) ] The facility is using green shell mussels and clams for seafood soups. The person in charge did not have proper shell stock tags or packages for these items. Shell stock that is raw or undercooked before cooking (mussels, clams, and oysters) must have proper shell stock identification tag present so item can be properly traced.  New Violation. 11A, Violation of Code: [.04(6)(e) ] Items such as soup and beans found cooling in deep container in cooler and stove top. The person in charge discarded the beans as they did not make proper cooling temperature and the soup was moved to more shallow containers and ice bath to finish cooling. Potentially hazardous foods must be properly cooled by effective methods (shallow pans, ice water bath, adding ice as an ingredient, etc.).  Corrected On-Site.  Repeat Violation. 12C, Violation of Code: [.04(4)(m) ] In use wet wiping cloths stored in chlorine sanitizer that was below 50ppm. The person in charge removed the cloths and remade the sanitizer to the proper concentration of 50-100ppm. All in use wet wiping cloths must be stored in proper sanitizer between uses (50-100ppm chlorine or 200-400ppm quat sanitizer).  Corrected On-Site.  Repeat Violation.
