Violations: 4-2B, Violation of Code: [.05(7)(b) ] Table cleaned one time by employee with cloth towel. Had employee return to table to clean via two step method. Dining counters/table-tops shall be cleaned and sanitized routinely after removing all soiled tableware after each use by one of the following methods or other method approved by the Health Authority: (i) A two step method may be used in which one cloth, rinsed in sanitizing solution is used to clean food debris from the surface and a second cloth in separate sanitizing solution is used to rinse; or (ii) Sanitizing solution is sprayed onto the surface and the surface is then wiped clean with a disposable towel.  Corrected On-Site.  New Violation. 12D, Violation of Code: [.04(4)(g) ] Heads of lettuce cut prior to washing. Employee took head of lettuce out of cardboard box and cut through it. Except as specified in 2 of this subsection and except for whole, raw fruits and vegetables that are intended for washing by the consumer before consumption, raw fruits and vegetables shall be thoroughly washed in water, in a sink designated for that purpose only, to remove soil and other contaminants before being cut, combined with other ingredients, cooked, served, or offered for human consumption in ready-to-eat form.  New Violation. 13A, Violation of Code: [.10(2)(g) ] Health score of an 83 posted that was not officially issued during the last inspection. The health score of an 83 was amended to a 79 & a 79 was the final score that was issued to the manager on 10-09-2013. Manager was informed at that time that the score had been amended to a 79 & was issued that updated inspection report. VOIDED out the 83 that was posted. The health score was also posted behind the service counter. The most recent health score must be posted for public view within 15 feet, 5-7 feet from the floor AND readable within 1 foot.  New Violation. 15A, Violation of Code: [.05(1)(i) ] Lint build up above the dishwasher. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.  New Violation.

Violations: 2-2B, Violation of Code: [.03(5)(j)1&2 ] Open container with beverage stored above food; a water bottle & 4 containers with beverage in single use cup with a lid but without a straw noted throughout kitchen. Had facility transfer water to single use cup with a straw; had facility place lids and straw on other drinks; had facility relocate dinks to approved area. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. IF THIS ITEM IS MARKED OUT OF COMPLIANCE ON THE NEXT ROUTINE INSPECTION, THEN THE PERMIT MAY BE SUSPENDED.  Corrected On-Site.  Repeat Violation. 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Washed/cut vegetables stored below unwashed vegetables. Raw pork stored with cooked chicken in freezer. Had facility relocate unwashed produce below washed/cut produce. Had facility separate raw pork from cooked chicken in freezer. Must store unwashed produce separately from ready to eat food & washed/cut produce. Must store raw meats separately, below ready to eat foods.  Corrected On-Site.  New Violation. 10D, Violation of Code: [.04(4)(d) ] Bulk products removed from their original packages, and not labeled with the common name of the food. Multiple bulk food items on cart by wok not labeled. Had facility label product at time of inspection. Except for containers holding food that can be readily and unmistakably recognized such as dry pasta, working containers
holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cook- ing oils, flour, herbs, potato flakes, salt, spices, and sugar shall be identified with the common name of the food.  Corrected On-Site.  New Violation. 15B, Violation of Code: [.05(6)(d),(e) ] Dishwasher not dispensing chlorine. Dishwasher must operate as per manufacture's instruction.  New Violation.

Violations: 3-1A, Violation of Code: [.04(2)(a) ] Receipts for all foods were not available to verify food sources. Find receipts and keep a copy of them at the facility at all times. Food must be obtained from sources that comply with law.  New Violation. 5-1B, Violation of Code: [.04(5)(f) ] Brown stew chicken was placed on hot holding unit to reheat. Item did not reach 165 degrees Fahrenheit within 2 hours. Item was discarded. Food that is being reheated for hot holding must be reheated within 2 hours to 165 degrees Fahrenheit. Note: Hot holding units (i.e. steam baths) are not appropriate for reheating food. Reheat food on stove or in oven first to 165 degrees Fahrenheit, and then transfer food to hot holding unit.  Corrected On-Site.  New Violation.
