Observed containers (strainers, bowls, lids) stored as clean next to fryers with dried food debris. Drink nozzles cleaned and sanitized. Ice machine and dirty equipment needs to be cleaned and sanitized to be free of mold/mildew/soil accumulation. Food contact surfaces should be kept free of mold/mildew. THIS IS A REPEAT VIOLATION. IF THIS VIOLATION IS MARKED OUT ON THE NEXT INSPECTION PERMIT IS SUBJECT TO SUSPENSION.  Corrected On-Site.  Repeat Violation. 6-1D, Violation of Code: [.04(6)(i) ] Sushi Rice labeled with a discard time of 3:00 PM was still located on sushi prep line at 3:25PM. Rice was discarded. All sanitizer diluted to the proper concentration. Chemical sanitizer should be available at the concentration level recommended by the manufacturer.  Corrected On-Site.  New Violation. 10D, Violation of Code: [.04(8)(b) ] Two container of Heavy Cream with use by dated of 10/5/2013. Cream was discarded. Shrimp moved to cooled. Potentially hazardous foods should be thawed using an approved method (in a cooler 41oF or below, under running water no more than 70oF, or cooking).  Corrected On-Site.  New Violation. 12B, Violation of Code: [.03(5)(h) ] Employee aprons being stored over food and equipment on hibachi carts. Items removed from carts. Employee clothing should not be stored in contact with food/food contact equipment.  Corrected On-Site.  New Violation. 14A, Violation of Code: [.04(4)(k) ] Spoons at sushi area stored in water a 70oF. Items took to be washed. If utensils are stored in water the water must be kept at 135oF or below. Otherwise utensils should be stored in a clean container and washed, rinsed, and sanitized every 4 hours.  Corrected On-Site.  New Violation. 15A, Violation of Code: [.05(1)(a) ] Cloths, napkins, and paper towels being used in contact with food. Remove these items from food container. Bottom of 2-door upright cooler with dried food debris. Counters next to grill with grease build up. Clean hand sink, next to grill, and cooler.
