IF THIS IS A REPEAT ON THE NEXT ROUTINE INSPECTION PERMIT IS SUBJECT TO SUSPENSION. Repeat Violation. 2-2A, Violation of Code: [.03(4)(a)-(f) ] Person in charge was not able to identify the 5 symptoms/ illnesses of the employee health policy, when to exclude/restrict, and what to report to the Health Department. PIC was provided a handout of the employee health policy. Must ensure preparation sink is designated as a vegetable preparatin sink and only fruits and vegetables are washed in sink. Sink cleaned and sanitized at time of inspection. Corrected On-Site.
