Violations: 6-1D, Violation of Code: [.04(6)(i) ] Sliced cheese placed under time control with label of 10:00 am. The temperature of the cheese taken at 10:00am was 46F. According to employee, cheese was actually placed on prep line at 9:45 and the time was rounded up to 10:00am. Labeled was changed to correct time of 9:45am. Items placed under time control must be 41F or below at the time placed on time control. Put time cheese was pulled from walk-in cooler as start time for time control. Do not round up time.  Corrected On-Site.  New Violation. 8-2B, Violation of Code: [.07(6)(g) ] Sanitizer buckets under prep line tables tested above 200ppm. Buckets were emptied and new buckets were made of proper concentration. Chlorine sanitizer should be between 50-100ppm. Chemical sanitizers and other chemical antimicrobials applied to food-contact surfaces shall meet the requirements specified in 40 CFR 180.940.  Corrected On-Site.  New Violation. 13A, Violation of Code: [.10(2)(g) ] Current inspection report is not displayed. Previous inspection report with score of 98 is displayed. Display current report. The most current inspection report must be displayed within 15 feet of front door, 5-7 feet from floor, and able to be read from 1 foot away.  New Violation. 14D, Violation of Code: [.04(4)(n) ] Observed employee putting chicken in plastic container. Employee dropped container on floor, picked container up and placed it in sink, and then touched chicken with same gloved hand. Chicken was discarded, hands were washed, and gloves were changed. Gloves must be changed whenever they become contaminated.  Corrected On-Site.  New Violation.

Violations: 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Observed onions horizontally in front of raw chicken, causing raw chicken to be lifted over and across onions when removing chicken from cooler. Onions were moved behind chicken. Foods must be stored in a manner that prevents cross-contamination.  Corrected On-Site.  New Violation. 4-1A, 1. Tongs to gather raw chicken from prep cooler were stored directly on top of chicken with handle in contact with raw chicken. Tongs were removed. 2. Scoop to gather raw chicken wings from container was stored directly in container with handle coming in contact with raw chicken. Observed emoployee touch contaminated handle to gather raw chicken and then touch equipment and continue to handle foods. Scoop was removed from container to be washed. Store tongs/scoops, etc. in a manner that prevents the handle from coming in contact with raw animal products. Food, food-contact surfaces, and equipment must be protected from cross-contamination at all times.  Corrected On-Site.  New Violation. 17D, Violation of Code: [.05(3)(d) ] Observed grease hood with grease build-up that could possibly drip down into food. Increase frequency of cleaning to prevent build-up. Grease hoods must be cleaned with adequate frequency to prevent the build-up of grease and condensation.  New Violation
